People with disabilities are often times living in nursing homes when they want and are capable of living more independently in their communities. Self Reliance provides assistance to such individuals enabling them to choose a living arrangement of their choice. Each person’s situation dictates the process and timeline for transitioning to a more independent living arrangement. This process includes an assessment of the needs of the consumer including daily living, social, medical/nursing, support system including families, friends and professionals, financial needs and resources. The consumer defines their needs, wants, desires and goals. Many times the resources and supports necessary to independent living take time to be identified, obtained and utilized to meet the individual needs of the consumer.

 If you or someone you know lives in a nursing home but wants to move back to the community please call the Clearinghouse on Disability at 866-273-2273 and tell them you want information on the Nursing Home Transition Program.

 You may be eligible for the ADA Medicaid Waiver Program if: 

  1.        You are between the ages of 18 and 59
  2.        You are eligible for Medicaid
  3.        You reside in a nursing home for 60 consecutive days
  4.        You can live safely in the community with support services

 If you do not qualify for the ADA Medicaid Waiver Program Self Reliance can assist you in assessing your needs, identifying available resources and helping you build a support system necessary for living in a community and home of your choice.
